Growth hormone and IGF-1 can be changed pharmacologically. The useful question is what that change accomplishes, for whom, and at what cost. The clearest human examples include meaningful benefits in a particular disease, increased fat-free mass without better function, and a large trial in which the hoped-for clinical benefit did not appear.
These are related interventions, not interchangeable names
| Intervention | What it is | Evidence question that matters |
|---|---|---|
| CJC-1295 with DAC | A modified growth-hormone-releasing hormone analog with prolonged albumin binding | Do sustained hormone changes improve a relevant clinical outcome? |
| Products described as CJC-1295 without DAC | A different exposure from the long-acting compound in the original trials | Does the exact product have its own evidence? |
| Ipamorelin | A peptide ghrelin-receptor agonist | Does receptor stimulation improve the intended outcome, beyond releasing GH? |
| Tesamorelin | A growth-hormone-releasing factor analog with an approved, specific indication | Does the population match the studied and approved use? |
| Ibutamoren / MK-677 | An orally active non-peptide ghrelin mimetic | Do changes in IGF-1 and body composition translate into function? |
FDA’s CJC-1295 evaluation distinguishes the long-acting DAC forms from products without DAC. A familiar abbreviation is insufficient to establish identity or transfer a study’s findings. FDA characterizes ibutamoren as a small chiral molecule; selling it alongside peptides does not turn it into one. FDA CJC-1295 evaluation, FDA ibutamoren evaluation
CJC-1295: sustained exposure was demonstrated
Two randomized, placebo-controlled ascending-dose studies lasting 28 and 49 days studied healthy adults aged 21–61. The long-acting compound increased mean GH concentrations approximately twofold to tenfold for at least six days and IGF-1 approximately 1.5-fold to threefold for nine to eleven days after a single research exposure. No serious adverse reactions were reported in those trials. The main outcomes were hormone concentrations and pharmacokinetics, rather than tendon healing, training performance or durable strength. Original 2006 trials
A related physiological experiment found that pulses persisted, while trough GH rose substantially. Preserved pulsatility therefore does not mean the hormonal environment stayed unchanged. Nor does a pulse pattern establish that a pharmacological exposure reproduces the benefits and risks of ordinary physiology. Original pulsatility study
The short studies provide evidence of biological activity, with a limited safety window. FDA’s current compounding-risk discussion also identifies increased heart rate and systemic vasodilatory reactions among reported serious adverse events associated with CJC-1295. That broader information matters alongside the favorable early trial account. FDA compound-risk information
Ipamorelin: a human trial, but a different claim
Ipamorelin has been tested in humans, including a randomized postoperative bowel-resection trial. Of 117 enrolled patients, 114 were included in the safety and modified intention-to-treat populations. Median time to a tolerated meal was 25.3 hours with ipamorelin and 32.6 hours with placebo, but the difference was not statistically significant; neither the key nor secondary efficacy analyses established a benefit. This tested postoperative intestinal recovery, not lifting performance or shoulder repair. Original randomized trial
The paper described treatment as well tolerated. FDA’s fuller evaluation notes two deaths in the treated group, occurring in complex postoperative circumstances, and explicitly states that their relationship to ipamorelin was unclear. The accurate interpretation preserves both facts: deaths occurred during the trial, and the available evidence does not establish that the drug caused them. Small, short studies cannot settle long-term safety in healthy users. FDA ipamorelin evaluation
Ibutamoren: more fat-free mass did not establish better function
A trial in healthy older adults randomized 71 participants; 65 completed the first year. The one-year mean change in fat-free mass was +1.1 kg with MK-677 versus −0.5 kg with placebo. Strength and function did not improve. The study was too small to exclude every functional benefit, but its positive body-composition outcome cannot be substituted for a demonstrated improvement in function. Fasting glucose increased and insulin sensitivity decreased; increased appetite, edema and muscle pain were reported. The two-year analyses were follow-up of this cohort, not a second independent trial. Original older-adult trial
Fat-free mass includes water and other non-fat tissues. That trial also measured changes in intracellular water. A gain in fat-free mass should therefore not be reported as an equal gain in contractile muscle.
A separate trial randomized 563 people with Alzheimer disease. IGF-1 rose markedly, but the intervention did not slow deterioration on the cognitive, global or daily-function outcomes assessed over twelve months. This is a particularly clear example of target engagement without the intended clinical benefit; it does not establish an effect on cognition in healthy young adults. Original Alzheimer trial
A hip-fracture study in 123 older patients found a favorable gait-score signal while its stair-climbing power outcome and several other functional measures did not improve significantly. The study was stopped early because of a congestive-heart-failure signal. FDA’s evaluation reports four cases among 62 treated participants versus one among 61 receiving placebo. That is consequential counterevidence in this vulnerable population, without providing a precise risk estimate for every potential user. Original hip-fracture trial, FDA ibutamoren evaluation
Tesamorelin: real evidence with a specific clinical boundary
Tesamorelin has an FDA-approved use: reducing excess abdominal fat in adults with HIV-associated lipodystrophy. The label explicitly says it is not indicated for weight-loss management, describes it as weight neutral, and notes that long-term cardiovascular safety has not been established. Approval for that indication does not establish a general rejuvenation or training-recovery treatment. EGRIFTA WR prescribing information
In one pivotal randomized trial of 404 people with HIV and excess abdominal fat, visceral adipose tissue declined 10.9% with tesamorelin versus 0.6% with placebo over six months. The extension followed the same participants: continued treatment maintained benefit, while switching to placebo rapidly lost the initial reduction. The extension adds information about persistence and withdrawal, rather than independent replication. Original trial and extension
A separate twelve-month trial enrolled 61 people with HIV and fatty liver. The between-group effect on hepatic fat fraction was −4.1 percentage points, with a 95% confidence interval from −7.6 to −0.7. This supports a particular liver-fat effect in that population; it does not establish broad prevention of liver disease in healthy people. Original liver-fat trial
The prescribing information also includes increased IGF-1, fluid retention, glucose intolerance or diabetes, hypersensitivity and injection reactions among relevant concerns. Active malignancy is a contraindication. An approved drug can have a favorable balance for its intended use while remaining inappropriate for another purpose. EGRIFTA WR prescribing information
What would change my assessment?
For my training question, the missing bridge is controlled evidence in a relevant population measuring useful recovery, strength or function, with a meaningful safety window. An improved IGF-1 result cannot supply that bridge by itself. The recovery discussion covers the current shoulder context, and the nutrient guide keeps dietary adequacy separate from pharmacological experimentation.
Regulatory status also needs a date and a precise claim. Ibutamoren remains listed in FDA’s Category 2 nomination document, while other substances appear in different or withdrawn-nomination sections. These are compounding-policy categories, not FDA approval. This investigation describes research; it does not provide a drug regimen. FDA nomination categories, May 2026
