Incretin drugs: weight, muscle and useful outcomes
In this investigation
These drugs are an unusually strong example of peptide biology becoming effective medicine. They also show why the outcome matters: lower body weight, less fat, preserved strength and fewer cardiovascular events are related but separate questions. My current interest is understanding the frontier. My food log and training reports do not establish an indication for weight-loss medication.
Similar names, different interventions
Semaglutide activates the GLP-1 receptor. Tirzepatide activates GIP and GLP-1 receptors; retatrutide adds glucagon-receptor activity. That third target does not automatically make a drug better for every purpose. Comparisons require appropriate populations, outcomes and preferably a direct comparative trial. FDA semaglutide label, FDA tirzepatide approval, Original retatrutide phase 3 trial
Approval is product and indication specific. Wegovy has weight-management and other defined indications, and its 2026 label includes injectable and oral formulations. Zepbound has an obesity-associated sleep-apnea indication as well as weight management. These are prescription medicines with substantial human development programmes. Retatrutide remains investigational at this review date. FDA semaglutide label, FDA tirzepatide approval, FDA current unapproved-drug statement
The scan measures lean tissue, not strength
DXA estimates fat and lean soft tissue. The latter includes muscle, organs and water and excludes bone mineral content. A fall in DXA lean mass is therefore not a measurement of an equal amount of lost contractile muscle. Conversely, an improved lean-to-fat ratio can coexist with an absolute fall in lean mass. Neither ratio nor scale alone establishes preserved function. Body-composition definitions in the original SURMOUNT-1 substudy
| Original body-composition study | What was observed? | Important boundary |
|---|---|---|
| STEP 1 semaglutide substudy; 140 participants, 68 weeks | Fat mass fell 19.3% and lean mass 9.7% in the semaglutide arm; lean mass as a proportion of body weight rose | Exploratory conference report from the parent trial; predominantly women with overweight or obesity, not trained young men |
| SURMOUNT-1 tirzepatide substudy; 160 paired scans, 72 weeks | Fat mass fell 33.9% and lean mass 10.9%, versus 8.2% and 2.6% with placebo; approximately 75% of lost weight was fat and 25% lean in both arms | These percentages describe different compartments, not fractions to add; 160 analyzed participants are not the full 2,539-person trial |
| Retatrutide phase 2 diabetes substudy; 103 paired scans, 36 weeks | Substantial fat reduction; the authors described the fraction of weight loss from lean tissue as similar to other obesity treatments | Participants had type 2 diabetes; the inspected abstract does not establish superior muscle preservation or give a strength outcome |

The practical interpretation is to assess body composition together with strength, mobility, symptoms and clinical benefit. A 25% lean share in both groups does not mean that their absolute lean losses were equal. Nor does a population average guarantee a favorable outcome for someone already lean, underfed or frail.
Clinical benefit can improve while weight is lost
In SELECT, 17,604 adults aged at least 45 with established cardiovascular disease, overweight or obesity, and no diabetes were randomized to semaglutide or placebo. Major cardiovascular events occurred in 6.5% versus 8.0%; the hazard ratio was 0.80 (95% CI 0.72–0.90) over a mean 39.8 months of follow-up. That is evidence about important health events in a defined high-risk population, not a general longevity guarantee. Original SELECT trial
Measured function can improve too. In 529 people with obesity and heart failure with preserved ejection fraction, semaglutide improved six-minute walking distance relative to placebo by 20.3 m (95% CI 8.6–32.1) at 52 weeks. Walking with heart failure is a different endpoint from maximum strength or sprint performance, but this trial prevents the simplistic conclusion that weight loss must mean worse function. Original STEP-HFpEF trial
Retatrutide: the newer evidence and the headline discrepancy
The September 29, 2026 TRIUMPH-1 report randomized 2,339 adults with obesity without diabetes for 80 weeks. Treatment-regimen weight changes were −17.6%, −23.7% and −25.0%, versus −3.9% with placebo, estimating effects regardless of adherence or prohibited weight-management treatment. Knee-pain and sleep-apnea outcomes improved in relevant subgroups; gastrointestinal events were most common. Original report.
The sponsor’s 28.3% highest-dose efficacy estimate instead assumes continued treatment without prohibited therapy. Its 30.3% at 104 weeks describes a selected extension of completers who tolerated treatment and met further eligibility restrictions. Neither replaces the 25.0% primary estimate; the extension is the same trial. Highest-dose adverse-event discontinuations were 11.3% versus 4.9% with placebo, and altered skin sensation 12.5% versus 0.9%. Sponsor results and definitions.
A strong trial does not establish the identity, sterility or exposure of a separately marketed vial. FDA currently states that retatrutide is not approved and cannot be used in compounding under federal law. The phase 3 result and this regulatory status can both be true. FDA current statement
Exercise remains a separate contribution
In the S-LiTE trial, 195 adults with obesity were randomized after an initial eight-week diet to exercise, liraglutide, both, or placebo for a year. Combining exercise and medication produced the largest improvement in body-fat percentage; only the combined strategy improved the reported cardiorespiratory-fitness endpoint. Its additional weight loss was significant versus exercise alone, but the direct comparison with liraglutide alone was not statistically significant. This supports measuring complementary outcomes rather than treating appetite suppression as a replacement for training. It does not identify an optimal training or protein prescription for every newer drug. Original exercise–liraglutide trial
Durability and tolerability belong in the decision
The STEP 1 withdrawal extension followed 327 participants from the original trial. A year after semaglutide and the lifestyle intervention stopped, the former semaglutide group had regained approximately two-thirds of its prior weight loss. Because both interventions stopped and this was an exploratory selected extension, it cannot isolate a drug-only withdrawal effect or prove that everyone needs lifelong treatment. It does show why the maintenance plan and follow-up matter. Original STEP 1 extension
The semaglutide label includes pancreatitis, gallbladder problems and kidney injury associated with dehydration among important warnings. Its thyroid warning arises from rodent findings; the human relevance remains unknown, and personal or family medullary thyroid carcinoma and MEN2 are contraindications. These specifics are more informative than saying peptide medicines are either universally safe or universally dangerous. FDA semaglutide label
For my training questions, the nutrition guide and recovery investigation remain directly relevant. The next pharmacological research question is whether a muscle-directed adjunct can preserve useful function during indicated weight loss—not just make a scan look better.
