Spermidine: exposure, autophagy and human outcomes
In this investigation
Spermidine is a polyamine. Its human trials need the measured outcome kept clear: blood exposure, immune-cell assays, memory or physical function. I have not reported taking it.
An oral dose does not identify tissue exposure
A randomized crossover in 12 healthy adults tested a commercial algae/soy preparation supplying 15 mg/day for five days, against placebo, with a nine-day washout. All completed; meals were standardized on sampling days. Plasma spermidine did not significantly increase. Spermine, a related polyamine, showed nominally higher integrated plasma exposure (P=.0282); saliva measurements were unchanged. The study tested pharmacokinetics, not memory, autophagy, training gains or longevity. It suggests metabolism matters, without proving that unchanged plasma levels mean no tissue effect or that a larger dose is beneficial. Authors declared no conflicts or external funding. A new scan report does not change the primary memory result
SmartAge randomized 100 adults aged 60–90 with subjective cognitive decline to twelve months of wheat-germ extract, approximately 0.9 mg spermidine daily, or placebo; 89 completed. The primary memory result was null: adjusted effect −0.03, 95% CI −0.11 to 0.05.
An exploratory MRI report published online 9 October 2026 uses the same cohort. Seventy-five had paired scans; 56 entered the perfusion comparison. Selected regional perfusion and white-matter findings were positive, while global measures and associations with memory change were not. Multiple uncorrected tests make these preliminary signals. The supplement supplier provided product/development support; some authors disclosed equity and advisory roles. This is one trial, not independent replication. Primary report, 2022, new MRI report
Immune-cell findings deserve their experimental context
A 2026 forty-person safety/feasibility pilot reported 6 mg/day of wheat-germ-derived spermidine for thirteen weeks after COVID vaccination; its baseline table includes 38 people. Overall anti-spike IgG change was not significant. Prominent antibody findings concentrated in eight baseline nonresponders; placebo had only two. RNA sequencing selected five nonresponders with the largest antibody improvements. B-cell autophagic-flux assays showed a signal, but these analyses do not establish fewer infections or slower aging. Numerous tests, subgroup imbalance and unclear participant accounting limit inference. The supplier provided product and funding; authors disclosed consultancies. Alsaleh and colleagues, 2026
The cardiac and physical-function trial remains a question
The POLYCAD protocol reports 187 randomized adults aged at least 65 with coronary disease, testing 24 mg/day against placebo for 48 weeks. Four primary outcomes concern heart-muscle mass, peak oxygen uptake, appendicular lean mass and inflammation. Its June 2026 analysis plan explicitly separates these domains, with no multiplicity adjustment or overall success criterion. It is not powered for major cardiovascular events. DoNotAge donated capsules; investigators report no advisory/financial relationship with it.
The current registry marks the study completed in June 2026, with no posted results at this review. A focused search did not identify its final functional outcome report. These are a protocol and registration, not demonstrated benefit. Original protocol, 2025
What this changes here
These research exposures establish no extra purchase for my routine. Different products, populations and measured outcomes remain distinct.
The longevity investigation and trial-reading notes develop the broader decision.
